This survey is the direct result of research into creativity and innovation in 1995 by The Centre for Creativity(1) and Innovation that was refined in 2002 by The Learning Cup Ltd. The questionnaire should take less than 10 minutes to complete. There are no right or wrong answers and it is best not to labour too much over any question. Your first thoughts are probably best.

On completing the survey will immediately obtain a short report based on your answers to give you insight into:

• Your total creative potential in terms of your creative personality, your problem solving approach and the creative environment you work in;
• Your organisation's environment for innovation;
• Your overall organisational environment, in terms of its philosophy, use of staff, view of its market and processes.
